3/14/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, QMHP-C engaged mother in a discussion to determine behaviors since the last session. Ms. Broadnax reported that Anthony go into a verbal confrontation with several family members last night over a disagreement with a family member. Ms. Broadnax reported that Anthony continues to exhibit at-risk behaviors within the home, school and community. . Ms. Broadnax reported that he was being disrespectful and loud after given instructions to stop turning the television. 2.Behavior: Ms. Broadnax reported that Anthony became angry and upset with his prior to QMHP-C arrival for todays. Ms. Broadnax reported that Anthony stated “Fuck Them and You for taken their side”. Ms. Broadnax went on to report that Anthony has serious issues dealing his disrespectful behaviors. 1.Intervention: QMHP-C assessed and monitored Anthony’s mood and behavior. QMHP-C probed in order to process and explore Anthony’s poor communication skills used with his parents and others. QMHP-C reviewed and discussed effective ways to communicate with others when provoked or angry within the home and community. QMHP-C provided Anthony with note card to assist his with brainstorming techniques to assist his with decreasing his behaviors within the neighborhood. 2.Intervention: QMHP-C reviewed the importance of trying to meet goals and objectives with Anthony. QMHP-C asked questions about reported behaviors within the home. QMHP-C allow Anthony to ask questions along with expressing his feelings and concerns. QMHP-C and Anthony reviewed and discussed anger management techniques. QMHP-C provided Anthony with feedback along with demonstrating model techniques to utilize when provoked or felling angry or depressed. 1.Response: Anthony stated that he was upset and didn’t feel like hearing or seeing his family members who live with her. Anthony was observed being disrespectful to his parents during todays session. Anthony continue to talk back towards Ms. Broadnax while talking to QMHP-C. Anthony went on to report that he doesn’t like everyone teaming up on his when it comes to his family members. Response: Anthony admitted that he was a little disrespectful and stated he didn’t care. Anthony stated that he will talk to QMHP-C later about some issues dealing his relationship with his dad. Anthony stated that he will just stay to himself and call his friends later on discuss homework assignments.
